- Comprehensive outpatient care
- Counseling on managing chronic conditions such as diabetes, heart disease, and hypertension
- Physical examinations - school, sports, camp, drivers
- Immunizations and vaccinations
- Minor procedures - wart removal, skin biopsies, etc.
- Well child checks
- Gynecological care including routine pap smears and contraceptive counseling
- Pre-operative exams and cardiac testing including EKG’s
- Pulmonary function tests
- Motor vehicle accidents
The following are some basic guidelines concerning our policies and billing procedures.
Charges: Please feel free to discuss our fees with the billing manager at any time. We want you to understand our charges and feel satisfied that they are reasonable and fair. If you have any questions about your account, please call. A phone call will often prevent a misunderstanding.
Insurance Cards: Please present your insurance card(s) at every visit. Please notify the receptionist if your insurance has changed and/or you have a secondary insurance.
Payments: All payments are collected at the time of service. A $5.00 surcharge will be added if payment is not received at time of service.
Deductible: Payment of your deductible is due within 30 days of the billing date. This represents your responsibility for services as indicated by your policy. Please call your insurance company if you have questions regarding your deductible.
Insurance Processing: Your insurance company should make payment on your claim within 60 days. If your carrier fails to respond or makes a partial payment, you will be billed the balance. We will do everything we can to assist you in getting payment from your insurance carrier.
Missed Appointments: There will be a $25 charge for missed appointments.
Delinquent Bills: Failure to respond to multiple billing notices and comply with billing arrangements may result in account suspension for delinquency. We will make every effort to work with you in special circumstances. We cannot work with you, however, if we cannot reach you prior to your appointment.
Patient Record Copies: After receiving a signed authorization form to release medical records, we will be happy to have the copy service make copies of your records. There is a charge for this service and you will receive an invoice from the copy company.
Forms: We will be happy to have our clinical staff complete all appropriate and necessary forms. There is a $10 prepayment fee for this service. Please be sure any section to be completed by the patient is done so before dropping off the form. The form will be completed as soon as possible and you will be notified of completion. If you do not hear from us within one week, feel free to contact us. There will be no charge for any forms completed during an appointment
Please call at least three business days in advance so that we can be sure to have everything completed by your appointment. In order to request a referral, you must leave a message stating the reason for the referral (diagnosis), the ordering doctor’s name, and the type of visit or procedure to be done. Referrals are issued Monday through Friday during office hours. Referrals are sent electronically to specialists and we will call you to let you know the referral was completed.
Pre-Certifications: Some insurance companies require a pre-certification for certain types of tests (MRI, MRA, CT scans). It is the patient’s responsibility to know if a pre-certification is needed based on their particular insurance plan. If one is needed our office, or the specialist, must contact the insurance company to obtain the pre-certification. After requesting a pre-certification, it can take up to 48 hours for us to get a response from the insurance company. We will contact you once we receive the response.
Typically, you can expect routine tests to take three to seven working days. If special tests have been ordered, these may take longer. Results are given only to the patient, or parent if the patient is less than 18 years of age. You will not automatically be notified of your results. You may call for results at any time and they will be given to you. If a test was ordered by another physician or practice, you should call that office to obtain and discuss the results.
Please request long-term medication refills during office visits. If you need a routine refill between visits please contact your pharmacy a week before your prescriptions runs out. They will then send us a electronic refill request that the doctors can respond and send back to them electronically. Within 48 hours you are able to call the pharmacy to confirm the prescription has been completed.If your medication requires a written script please call the office and leave a detailed message. The office will call once the prescription has been completed. Written scripts can be picked up at our office during normal business hours. We will call you once the prescription has been completed.
